Design and implementation of capitation-based performance payments scheme at the Primary Health Care (PHC) level in Kosovo

The Government of Kosovo is supported by the World Bank to implement a Health Sector Project in the amount of US$25.5million. The Project aims at supporting the health sector reforms in Kosovo in order to increase (i) financial protection from the burden of health expenses, particularly among the poor; (ii) the quality of care and (iii) access to quality Maternal and Child Health (MCH) services and select cost-effective primary care interventions to prevent and manage Non Communicable Diseases(NCDs).

The project is built in three components:
1. Component 1: Improving financial protection and quality of care;
2. COMPONENT 2: Strengthening primary care;
3. COMPONENT 3: Project Management;

The design of capitation-based performance payments scheme sets the basis for implementing the second component of the Project that aims at strengthening primary health care. This component seeks to improve access to and quality of priority services at the primary care level, including MCH and NCD services, and therefore, to contribute to an enhanced sustainability of the mandatory health insurance reforms and improved population health outcomes. The component will finance capitation payments linked to performance. It is anticipated that these capitation payments conditioned on performance will strengthen incentives to improve access to and the quality of priority primary care services. The Ministry of Health (MoH) and the Health Financing Agency (HFA)will be responsible for managing the capitation payments (in addition to Municipal Government Health Financing Mechanisms). Once the MoH - HFA transitions its functions to the Health Insurance Fund (HIF), the HIF will be responsible for managing the capitation payments and will take over the HFA's role in the institutional arrangements related to the implementation of this component. Ultimately, it is envisaged that these payments will be financed through health insurance net revenues via performance agreements between the HFA/HIF and Municipalities, or directly between the HFA/HIF and primary care facilities. The Project may finance a proportion of these capitation payments up to the end of the Project, with Project financing tapering off by year 3 as the revenues mobilized by the HFA/HIF increase and can be channeled to the strategic purchasing of primary care services. The capitation-based performance payments scheme under the Project would cover all Municipalities in Kosovo, although implementation will be phased and participation by Municipalities is voluntary. Initially, capitation based performance payments will be piloted in Prishtina, Prizren, Malisheve and Dragash Municipalities, before being scaled up to cover other participating municipalities. The payments will cover only incremental expenditures required to improve access to and quality of primary care services, and has been determined to be an initial average amount of per capita by the MoH. As this is an average cost, it may be adjusted for the specific circumstances of individual Municipalities (for instance, remote areas would require more per capita resources) by the MoH - HFA in negotiations with individual Municipalities.

The objectives of this consultancy include the following: (i) Technical Assistance (TA) to the Ministry of Health and HFA/HIF to design the implementation arrangements of the capitation-based performance payments scheme under the Kosovo Health Project; and (ii) Training and other capacity building for HFA, Municipality and health facility staff in order to support the implementation of performance payments in Municipalities covered in the first phase of the implementation.

Scope of work:
The specific responsibilities include the following:
1. Develop administrative by-laws and regulations necessary to support the implementation of the capitation-based performance payments scheme and finalize a template for a Memorandum of Understanding between the MoH and MoF;
2. Develop the templates for performance agreements between the MoH/HFA/HIF and Municipalities;
3. Develop indicators to measure the performance of participating Municipalities and establish criteria and a scoring formula that rule the triggering of capitation-based performance payments to participating Municipalities;
4. Assist the HFA/MOH in setting a functional reporting measurement process of the percentage of achievements;
5. Design the institutional arrangements and standard operating procedures for all aspects of the implementation for the scheme;
6. Develop templates and formats to support the standard operating procedures where necessary. The standard operating procedures should be presented in a capitation-based performance payments manual;
7. Elaborate the roles of the key institutions involved in implementation, including the MoH, HFA, and Project Coordination Unit, the MoF, Municipalities and individual primary care facilities;
8. Assist with the development of Terms of References for the following entities involved in the implementation of capitation-based performance payments scheme;
9. Develop and overview plan for implementation in coordination with the relevant stakeholders;
10. Develop a training plan and training materials to support implementation in the phase 1 Municipalities, and lead trainings for MoH/HFA staff as well as for trainers in the four phase 1 Municipalities (both at health care facilities and the administrations of Municipalities);
11. Participate in discussions and meetings relevant to the objectives of the consultancy, including the relevant World Bank missions;
12. Carry out other tasks as can be reasonably expected.
